ENROLLMENT PROCEDURE FOR NEW/TRANSFEREE STUDENTS SY 2018-2019

Application/Testing

  1. Please take note of the following requirements:

    1. Age requirements
      • Pre-Kinder applicant must be 4 years old by June 2018 or at least 3 years and 10 months.
      • Kinder applicant must be 5 years old by June 2018 or at least 4 years and 10 months with prior schooling.
    • Grade requirements
      • Grade 1 applicants must be able to read short stories with comprehension
      • Grade in any subject/quarter must not be lower than 80 (Grades 1 10)
        • General Average: 83%
        • Conduct Average: 82%

Note: The Department reserves the right to deny applicants who have not met the required age/grade in the Final Report Card.

  1. Accomplish online application

(Click the link: __________________________________ ). Print the form and attach a 2x2 photo.

  1. Pay the testing fee at the Accounting Office
    • Pre-Kinder - Php 100.00
    • Kindergarten, Grades 1-5 - Php 150.00
    • Grades 6-8 - Php 120.00

Note: Incoming Grade 6 and 10 applicants must go to the Principal's Office before applying. Please bring photocopy of Form 138 (Report Card).

  1. Submit the following requirements to the Testing Center through personal visit or through courier:
  • Original Receipt (Testing Fee)
  • Printed online application form with a 2x2 photo
  • Photocopy of Report Card with latest grades for Grades 1-10
  • Photocopy of Birth certificate (for local, PSO Birth certificate)

The Testing Center will communicate with applicants after checking of the requirements via courier.

Note: Only applicants with complete requirements are allowed to take the test.

  1. Take the assessment as scheduled.
  2. Get the test results at the Testing Center on the date indicated on the stub that will be given to your child after taking the entrance test.
  3. Bring the test results and your child as scheduled to the
    • Vice-Principal for Academics for Pre-Kinder to Grade 6
    • Principal's Office for Grades 7 to 10

ENROLLMENT

  1. For successful examinees, accomplish online application (Click here: ___________________________)
  2. Submit the following requirements to the Principal's Office to secure enrollment:
    • Original copy of Final report card (Form 138)
    • Original copy of Certificate of Good Moral
    • Photocopy of PSO Birth Certificate
    • Photocopy of Baptismal Certificate
    • ID photos (2 pcs. of 1x1)
    • Medical Certificate (Pre-Kinder Grade 2)
  3. Proceed to the Registrar's Office and submit the approved printed online application form (this will also serve as a contract and to be submitted to the Principal's Office after enrollment).
  4. Pay the initial payment for enrollment of Php 2000.00 at the Accounting Office or through bank under the account name of Universidad de Sta. Isabel. Indicate your name in the deposit slip.

OLD STUDENTS

ENROLLMENT

  1. Accomplish online application

  1. Proceed to the Principal's Office to get the approved printed online application (this will also serve as a contract and to be submitted to the Principal's Office after enrollment). Please bring the original copy of Final Report Card.
  2. Pay the initial payment for enrollment of Php 2000.00 at the Accounting Office or through bank under the account name of Universidad de Sta. Isabel. Indicate your name in the deposit slip.
