ADMISSION POLICIES

I. GENERAL ADMISSION POLICIES

         The University admits students who are of good moral character and have the required academic credentials. Further, the admission will be based along the following:

  1. Results of the Entrance Examinations administered by the University Testing Center;
  2. Interview with the Executive Officer for Public Relations, Communication, Linkages, Marketing, Promotions and Admission and the Coordinator of the Religious Education Department for Non-Catholic Applicants; and
  3. Other pertinent requirements set by a particular college or unit.

II. ADMISSION POLICIES FOR FOREIGN STUDENTS

         The University admits foreign students who have the required academic credentials. More so, the student-applicants will be admitted based on the General Admission Policies and along the following:

  1. Non-English and/or Tagalog Speaking Student-Applicants should enroll in enrichment classes that will be administered by the pool of accredited tutors of the University; and
  2. Once the student-applicant is officially admitted by the respective college dean, he/she is not permitted to transfer to other college without the approval of the Coordinator for Admission.

III. REQUIREMENTS FORSTUDENTS' ADMISSION FOR SENIOR HIGH SCHOOL

A . Admission Requirements for Senior High School Applicants (Local)

  1. Original Junior High School Report Card (Form 138) with Learner's Reference Number (LRN);
  2. Original Certificate of Good Moral Character;
  3. Photocopy of Birth Certificate (NSO Certified);
  4. Photocopy of Baptismal Certificate;
  5. Three (3) copies of colored pictures (any background) - One (1) copy 2x2 and two (2) copies 1x1
  6. Voucher Certificate (Student Applicants from Private Schools)

B. Admission Requirementsfor Senior High School Applicants (Foreign Students)

  1. Original Junior High School Report Card (Form 138);
  2. Original Certificate of Good Moral Character;
  3. Photocopy of Birth Certificate (NSO Certified);
  4. Photocopy of Baptismal Certificate;

  C. Admission Requirements for Senior High School Transferees (Local)

  4. Photocopy of Baptismal Certificate;
  5. Three (3) copies of colored pictures (any background) - One (1) copy 2x2 and two (2) copies 1x1
  6. Special Study Permit for 17 years old below

I. REQUIREMENTS FOR STUDENTS' ADMISSION FOR COLLEGE

A . Admission Requirements for Freshmen Applicants (Local)

  1. Original Senior High School Report Card
  2. Original Certificate of Good Moral Character;
  3. Three (3) copies of colored pictures (any background) - One (1) copy 2x2 and two (2) copies 1x1
  4. Photocopy of Birth Certificate (NSO Certified);
  5. Photocopy of Baptismal Certificate;
  6. Photocopy of Marriage Certificate (Church Wedding) for married student applicants
  7. Medical Examination Certificate (CBC, Chest x-ray, Drug Test, Urinalysis, Hepa B Screening) for BSN, BSPT, BSMT and BSHM student applicants
  8. Certificate of Indigency(for unemployed) or Certificate of Employment (for employed) forMarillac Alternative Program (MAP) Applicants

B. Admission Requirements for Freshmen Applicants (Foreign)

  7. Valid VISA and Passport
  8. Alien Certificate of Registration Identification Card (ACRI Card)
  9. Clearance from the Bureau of Quarantine
  10. NBI Clearance

I. REQUIREMENTS FOR DEGREE HOLDERS

Degree Holders who are eligible for admission must submit the following credentials:

  1. Official Transcript of Records;
  2. Photocopy of Diploma;
  3. Photocopy of Birth Certificate (NSO Certified);
  4. Photocopy of Baptismal Certificate;
  5. Photocopy of Marriage Certificate (Church Wedding) for married students;
  6. Five copies of 1x1 picture (any background);
  7. Medical Examination Results (CBC, Chest X-Ray, Drug Test, Urinalysis and Hepa B) for Nursing, PT, ND, FSIM Students; and
  8. Student Visa and Permit to Study for foreign students.

VI. ADMISSION PROCEDURES

Procedures for Taking University Admissions Examination (UAE)

First Process: Pre-Enrollment

Please proceed to

  1. Proceed to the Office of the Admission to secure an application registration form. Fill out the form and attach one (1) piece of 2x2 coloredpicture and submit the completed application form to the Admission Staff for registration.

Admissions and Placement Office

  1. Proceed to the Accounting Cashier and for the payment of applicant's testing fee.

Accounting Office

  1. Present and submit to the testing center the registration form together with the testing fee receipt.

Testing Center

  1. Take the University Admission Examination (UAE) as scheduled (8:00 A.M. and 1:00 P.M.)

Enrollment Procedures

Second Process : Enrollment

Please proceed to

  1. Proceed to the Admission Office for the result of the UAE. Present admission requirements for review and secure the referral or recommendation notice from the Admission Office.

Admissions and Placement Office

  1. Proceed to the chosen college/program and present UAE result. for academic advising. Secure and fill out enrollmentform and submit to the Dean,Director or Program Chair for enrollment approval.

College Dean's Office/ Office of the Senior High School Director

  1. Submit admissions requirements and approved enrollment form to the Registrar's Office for Student Number and release of Assessment Form.

Registrar's Office

  1. Present Assessment Form to the Accounting Cashier for payment of Php.2,000enrollment fees.

Accounting Office

  1. Present the Enrollment Receipt to the MIS Office for picture-taking and processing of ID.

MIS

Third Process : Post- Enrollment

Please proceed to

  1. Proceed to the Admission Office for the schedule of the Summer Camp.

Admissions and Placement Office

  1. Secure referral slip for prescribed uniform cut.

 

  1. Attend the scheduled Summer Camp

 

  1. Report to USI on the first day of the opening of the school year.

 

  Procedures on Students' Re-Admission

Steps

Please proceed to

  1. Proceed to the Registrar's Office and request for applicant's Summary of Grades.

Registrar's Office

  1. Proceed to the Admissions Office and present the Summary of Grades and secure the Re-Admission Form.

Admissions and Placement Office

  1. Present the filled up Re-Admission Form to the College Dean's Office for the academic advising and approval of Re-Admission

College Dean's Office

  1. Submit the approved Re-Admission form to the Registrar's Office and proceed to online enrollment.

Registrar's Office

  1. Wait for the approval of the College Dean.

College Dean's Office

  1. When approved, proceed to the accounting office and pay the enrollment fee of at least Php2,000.

The University reserves the right to require all applicants to submit drug test results and other requirements from accredited testing agencies.

         All submitted credentials will be verified by the respective College Dean and the University Registrar to ensure authenticity and completeness. Thus, falsification of documents constitutes a ground for disqualification.
